• Recommended: Authorization for the Release of Financial Information (FERPA- Form #2)

Health Insurance

• Student Health Insurance: Waive or Enroll 
• Students are charged for the insurance EVERY YEAR
• All students must submit a valid waiver EVERY YEAR
• Enrollment/Waiver forms for the Fall 2025 opened late July 23rd
• Please refer to the Student Health Insurance page for details

• Make a one-time payment
• Set up a monthly payment plan
• International Student Payment Options
